Community Based Services is seeking a dependable and trustworthy Mental Health Program Assistant to join our team of caring individuals who take pride in the work and care they provide individuals. As a Mental Health Program Assistant you will play an important role in assisting the individual in achieving their goals. This includes understanding individual needs, supporting program plans and helping the individual reach their maximum potential. Responsibilities include: • Participating in the development, refinement and implementation of individualized program plans that will help individuals safely achieve their goals. • Assisting individuals in becoming valued community members by balancing individual goals, supporting good choices and facilitating relationships with support networks and community organizations. • Promoting skill development and independence with the individual to maintain optimal mental health, physical health and wellness. • Assisting individuals with household management tasks to ensure the home is meeting licensure requirements, household needs and safety. • Medication administration, assistance and management. • Contributing to the safety and security of the home, individuals served and co-workers. • Site: 4th Street Town: Morristown, MN • EASE Level: 2 • Shared Staffing: Minimum of 2 staff on site at all times. 4th Street provides services for up to four adults ages 18 and older of any gender at any given time. The program serves individuals with serious and persistent mental illness, co-occurring substance use disorders, and other significant mental health support needs requiring short-term stabilization services within the community. Due to the short-term stabilization nature of the program, admissions and discharges may occur frequently depending on the support needs of individuals being served. Services provided include medication management, transportation, symptom monitoring and support, assistance with activities of daily living, appointment support, mental health stabilization services, substance use support, behavioral support interventions, and coordination of care with treatment providers and community resources. *Site descriptions are subject to change at any time as individuals residing in the home or their level of care required may change.

PLEASE NOTE: Any time an employee bids, voluntarily transfers, or voluntarily demotes to fill a vacancy in an existing SOCS, any previously approved vacation may be rescinded if any employee already working in the SOCS has approved leave for any of the days.
