The Community Behavioral Health Hospital (CBHH) in Rochester is seeking a compassionate, person-centered Clinical Social Worker to join our team of dedicated individuals who are helping to make a difference in the lives of others. In this key role, you will deliver integrated health care and social services, along with participating as an active member of the interdisciplinary treatment team. Operated by the Minnesota Department of Human Services, the CBHH is a small (16-bed) inpatient psychiatric facility for adults experiencing acute mental health episodes. The length of stay is usually less than 60 days, during which treatment focuses on rapid assessment, stabilization, patient education, and discharge planning involving community support resources, with the overall goal of preventing future episode. Responsibilities of the Clinical Social Worker include: • Conduct comprehensive psychosocial assessments • Participate in group therapy • Meet one-on-one with patients weekly to provide support, psychoeducation, address barriers to discharge, and care planning • Facilitate patient and family education • Initiate appropriate referrals for post-discharge follow-up services • Maintain appropriate and effective relationships with community providers

Bachelor’s degree in psychology, social work, behavior analysis, or related field; and • Licensure/Certification in the State of Minnesota as a LSW • Two years of post-bachelor’s degree experience providing behavioral health clinical services including at least 3 of the 5 activities listed below:? o Delivering psychoeducation and/or behavioral interventions? o Providing case coordination among multidisciplinary team members? o Conducting comprehensive assessments? o Leading development of treatment/support plans as required by program licensure? o Planning discharge/transition and/or aftercare? OR • Master’s degree in psychology, social work, behavior analysis, or related field; • Licensure/Certification in the State of Minnesota as a LGSW or LICSW • One year of post master’s degree experience providing behavioral health clinical services including at least 3 of the 5 activities listed below:? o Delivering psychoeducation and/or behavioral interventions? o Providing case coordination among multidisciplinary team members? o Conducting comprehensive assessments? o Leading development of treatment/support plans as required by program licensure? o Planning discharge/transition and/or aftercare? OR • Master’s degree in psychology, social work, behavior analysis, or related field; and • Independent Licensure/Certification in the State of Minnesota (e.g., LPCC, LP, LICSW, BCBA, LMFT)
