As a Behavior Modification Assistant (BMA) you will be responsible for supporting individuals who are in our short- and long-term housing program. These individuals may have intellectual disabilities, mental illness, brain injury, and other emotional, behavioral, medical complexities. In this position you will ensure the people we serve achieve their highest quality of life and personal growth! You will do this by: • Assisting in assessing individual's needs by performing direct care duties • Developing and implementing residential and habilitation support plans with goals and methods. • Evaluating and documenting out-comes, maintaining or modifying the plans and performing related work as required. • Respond therapeutically using de-escalation strategies and positive behavior supports to individuals with challenging behaviors. Stacy, MN 55079 The home is located at 23963 Loyola St NE Stacy, MN 55079 approximately 3 miles west of Highway 35 N, on County Hwy 19. The site is a split entry home that can support 2 Individuals in two separate living spaces. All 3 Individuals must meet the Safety Net Criteria. There is an attached 2 car garage, and a shed in the backyard. This home is on a small city lot with neighbors on all sides, in a small housing development in rural Stacy. The lower living space includes 1 bedroom, 1 bathroom, a living area, a small kitchen, and office space. One male aged 32 resides in the lower level and presents behaviors that include verbal aggression, physical aggression, property destruction and self-harm. This male enjoys walks, swimming, bowling, watching movies on his ipad, and going to work. The upper level of the home includes 1 bedroom, 1 office space, bathroom, full kitchen/dining room, and a family room. There is one male individual aged 29 that lives upstairs and presents behaviors that include verbal aggression, physical aggression, and property destruction. The male individual enjoys electronic gaming, board games, cooking, shopping and other community events in the area. The female enjoys, crafts, cooking, shopping, animals, and community events in the area. The staffing is consistently scheduled to provide 2 staff to meet the needs of the individuals, typically with one staff upstairs, one staff downstairs and one float staff to work between both floors assisting as needed. There are 2 overnight staff, one up and one downstairs. Staff at the home need to be familiar with psychotropic medications, seizure protocol, and medication administration. Staff need to assist with activities of daily living.

• Experience working with individuals with intellectual disabilities, mental illness, and/or co-occurring diagnosis. • Experience with verbal de-escalation strategies and working with individuals who exhibit aggressive behaviors. • Clear communication skills to understand and communicate complex services with community members, employers and teams. • Must be at least 18 years of age. • Basic Math and computer skills. • Must possess and maintain a valid Driver’s License. • Must be able to perform all physical/essential requirements with or without reasonable accommodation(s).
