As a Behavior Modification Assistant (BMA) you will be responsible for supporting individuals who are in our short- and long-term housing program. These individuals may have intellectual disabilities, mental illness, brain injury, and other emotional, behavioral, medical complexities. In this position you will ensure the people we serve achieve their highest quality of life and personal growth! You will do this by: • Assisting in assessing individual's needs by performing direct care duties • Developing and implementing residential and habilitation support plans with goals and methods. • Evaluating and documenting out-comes, maintaining or modifying the plans and performing related work as required. • Respond therapeutically using de-escalation strategies and positive behavior supports to individuals with challenging behaviors. • Site Name: MSOCS 8th Avenue Site Address: Urban 2020 8th Avenue NE Owatonna, MN 55060 County: Steele County/MN Prairie County Alliance EASE Level: 4 Staffing Level: 24-hour awake staffing. Safe Emergency Staffing 1 staff to 3 individuals. Typical staffing: One individual has 1:1 staffing from 6 am-10 pm. During waking hours, the home has one or more additional staff depending on planned activities. Description: The 8th Avenue home is a 3-bed licensed home. It is a single-family dwelling, rambler style home with attached, single-car garage, and partially finished basement, located in a residential neighborhood (neighbors on all sides) on the north side of Owatonna. 8th Avenue is approximately ¼- ½ mile from 2 busy city streets. The home has 3 bedrooms, and 1 handicapped accessible bathroom located on the main floor. Main floor laundry is located near the bedrooms/bathroom. The kitchen, dining area and living room are centrally located and makeup the remainder of the 1st floor. The lower level (basement) contains one main room that is finished and used as additional living space for residents. The lower level also contains a ½-bath, 2 large, unfinished storage areas and an office. The home has concrete slab patio off the dining room and a moderate sized back yard. The home is completely enclosed by cedar fencing. The home is identified to support all females. Currently there are 3 women living in the home ranging in age from their 30’s to 40’s. Individuals supported may have been diagnosed with a variety of disabilities including intellectual disabilities in the mild to moderate range, Bipolar Disorder, Intermittent Explosive Disorder, Seasonal Affective Disorder (SAD), and Fetal Alcohol Syndrome Disorder (FASD). The home supports individuals with high behavioral needs which include deliberate self-harm, aggression (verbal and physical), property destruction, and elopement. Psychotropic medications in conjunction with behavior support programs, counseling, and specific behavior intervention plans are used to support individuals’ behavior management needs. Individuals also have other health diagnoses including seizure disorder and allergies. Staff provide medication management, assistance with all appointments, and care and support for completion of activities of daily living as indicated by each individual’s level of function. Individuals participate in vocational programs during the day. Individuals are supported in various community activities such as local Park and Rec programs, theater/plays, library, restaurants, shopping, and other local and area events. Social relationships are important to individuals in this home and time with friends in and out of the home is encouraged and facilitated. The home has 2 vehicles used to provide transportation to all activities and appointments.

• Experience working with individuals with intellectual disabilities, mental illness, and/or co-occurring diagnosis. • Experience with verbal de-escalation strategies and working with individuals who exhibit aggressive behaviors. • Clear communication skills to understand and communicate complex services with community members, employers and teams. • Must be at least 18 years of age. • Basic Math and computer skills. • Must possess and maintain a valid Driver’s License. • Must be able to perform all physical/essential requirements with or without reasonable accommodation(s).
