As a Behavior Modification Assistant (BMA) you will be responsible for supporting individuals who are in our short- and long-term housing program. These individuals may have intellectual disabilities, mental illness, brain injury, and other emotional, behavioral, medical complexities. In this position you will ensure the people we serve achieve their highest quality of life and personal growth! You will do this by: • Assisting in assessing individual's needs by performing direct care duties • Developing and implementing residential and habilitation support plans with goals and methods. • Evaluating and documenting out-comes, maintaining or modifying the plans and performing related work as required. • Respond therapeutically using de-escalation strategies and positive behavior supports to individuals with challenging behaviors. Westfield Site Description • Site Name: Westfield, Freeborn County • City/Location: Albert Lea, MN • MSOCS Westfield is located within the city limits of Albert Lea, situated in a quiet neighborhood setting just a short distance from Fountain Lake and local amenities. • Staffing level- 1:1 in the home, 2:1 in the community. • EASE Level: 4 • Age range: 30-40 years old • Staff provide behavior support by offering gentle redirection during periods of agitation, implementing individualized coping strategies when early signs of agitation or anxiety are observed (e.g., deep breathing, grounding techniques, taking space, engaging in calming activities, physical movement, positive self-talk, and use of structured routines), and reinforcing positive behaviors through consistent encouragement and support. • Staff support medication management by providing reminders to take prescribed medications as scheduled, monitoring for potential side effects, supporting blood glucose monitoring, and documenting medication adherence to promote safe and effective management of Type 2 diabetes. *Site description narratives are subject to change at any time as individuals residing in the home or program or their level of care required may change.
