As a Behavior Modification Assistant (BMA) you will be responsible for supporting individuals who are in our short- and long-term housing program. These individuals may have intellectual disabilities, mental illness, brain injury, and other emotional, behavioral, medical complexities. In this position you will ensure the people we serve achieve their highest quality of life and personal growth! You will do this by: • Assisting in assessing individual's needs by performing direct care duties • Developing and implementing residential and habilitation support plans with goals and methods. • Evaluating and documenting out-comes, maintaining or modifying the plans and performing related work as required. • Respond therapeutically using de-escalation strategies and positive behavior supports to individuals with challenging behaviors. • Site: MSOCS Forest Lake • Town: Forest Lake MN • Suburban setting: The house is located near the lake (Forest Lake), and several miles outside of the “town” part of Forest Lake. It is in a neighborhood where the houses are very close together. There are no businesses within walking distance. The house is about a block from North Shore Drive, which is a moderately busy road. • EASE level: 4 • Staffing Level: There are awake staff on site 24 hours per day. The awake overnight shift is 9pm-7am. There is currently one overnight staff on duty at a time. From 7am-9pm, the house is staffed with two- three staff from 7a-9p (7-3p, 11-7p, 1-9p, 3-9p). • Age Range: 30-57 years of age • Diagnoses: Schizoaffective Disorder, Psychosis, Moderate Intellectual Disability, Seizure D/O, Anxiety, Pseudobulbar affect, Conduct behavior, Downs syndrome. There are some challenging behavioral issues including physical aggression, property destruction. Some have high medical needs and need assistance with medical supplies and equipment and hygiene tasks. *Site description narratives are subject to change at any time as individuals residing in the home or program or their level of care required may change.

PLEASE NOTE: Any time an employee bids, voluntarily transfers, or voluntarily demotes to fill a vacancy in an existing MSOCS, any previously approved vacation may be rescinded if any employee already working in the MSOCS has approved leave for any of the days.
