As a Behavior Modification Assistant (BMA) you will be responsible for supporting individuals who are in our short- and long-term housing program. These individuals may have intellectual disabilities, mental illness, brain injury, and other emotional, behavioral, medical complexities. In this position you will ensure the people we serve achieve their highest quality of life and personal growth! You will do this by: • Assisting in assessing individual's needs by performing direct care duties • Developing and implementing residential and habilitation support plans with goals and methods. • Evaluating and documenting out-comes, maintaining or modifying the plans and performing related work as required. • Respond therapeutically using de-escalation strategies and positive behavior supports to individuals with challenging behaviors. • Site Name: Westfield • City/Location: Hayfield, MN • Rural or Urban setting: Rural, 8 miles outside of Blooming Prairie. • EASE level: 4 • Staffing ratio: 2:1 for 1 of the individuals in the community and 1:1 at the home. The other individual is 1:1 at the home and in the community with 24 hours awake staffing. Both the individuals work through a vocational day program. • Age range: 30-40’s • Prevalent diagnoses: Moderate ID, OCD, ADHD, HX of inappropriate sexually inappropriate behavior, antisocial personality, depression, impulse control, ODD, seizure disorder, anxiety. There is also some medication noncompliance. The individuals in the home are completely independent and ambulatory although one has bladder control issues. *Site description narratives are subject to change at any time as individuals residing in the home or program or their level of care required may change.

PLEASE NOTE: Any time an employee bids, voluntarily transfers, or voluntarily demotes to fill a vacancy in an existing MSOCS, any previously approved vacation may be rescinded if any employee already working in the MSOCS has approved leave for any of the days.
